Insurance & Superbills

At this time, A Mother’s Village, LLC is an out-of-network provider and does not bill insurance directly.

Payment

Payment is due at the time of service.

If a card is not stored on file, please submit payment at least 24 hours before your appointment to hold your session.

Superbills

Upon request, we provide a superbill with:

• CPT codes

• Diagnosis codes (when appropriate)

• Provider NPI

• Date and type of service

Submission of the superbill is the responsibility of the client.

Important:
Insurance coverage varies widely. Reimbursement is not guaranteed, and travel fees/surcharges are not reimbursable.
